I don't know why you are stuck on the "report" part....you want a paper...you want a case file...you want a sworn statement.
A report can be made with a telephone call, a letter, an email, etc. What the person or agency that gets that report does from their is on them, not the person making the effort to convey information. He contacted, and passed on info, thus making the report.
When a citizen tells an officer that a crime is happening somewhere, they are making a report to you. That is why if you do not act on it, you are not doing your duty.....Official Misconduct could be what someone could be looking at.
